What will the forms ask for?
The forms are for business enquiries. They will not request passwords, payment details, account access, patient information, legal files, customer lists or uploads.

How will Mindflow use the information?
Submitted information will be used to evaluate the request, provide the accepted check or answer the enquiry, prevent abuse, manage requested follow-up and keep the limited operating record needed for those purposes.
A form submission will not add you to a marketing list. Any future marketing choice must be separate and optional.
Who will have access?
Access will be limited to Youssef Hodaigui and a named provider or specialist who needs the information to operate the request.
Before collection starts, this page will identify the relevant hosting, form-delivery, email and security providers by role. Mindflow will review their access, data location, deletion options, terms and incident procedures.
Information may also be disclosed when law requires it or when reasonably needed to protect rights, security or the service from abuse.
How long will records be kept?

A narrower record may remain longer when a documented legal duty, dispute or security investigation requires it. The reason should be recorded.
What should you never send?
Do not submit passwords, authentication codes, platform credentials, payment information, customer or employee lists, patient details, legal-client information, privileged communications or confidential product and order records.
If accepted work later needs sensitive access, the signed scope will define a more appropriate channel and access rule.
What security controls will apply?
The public page, endpoint and provider handoff must use secure transport. Mailbox and provider access must use multi-factor authentication and named access.
Mindflow will test confirmation, routing, spam controls, failed delivery and deletion before activation. No site can promise absolute security, so the final notice will describe the controls actually in use.
Will the site use analytics or cookies?
No analytics or advertising tool is active in the audit preview.
Before production measurement begins, this notice will name the tools, explain their purpose and state the relevant consent or opt-out choices. Hosting and security providers may keep technical access logs; the final notice must describe those records accurately.
Form values and message bodies must not enter analytics events or routine application logs.
What choices will a person have?
Applicable law may provide rights to access, correct, delete, restrict or object to certain uses of personal information.
The final notice will publish a tested privacy contact and explain identity verification. Mindflow may keep information where law requires it or where a valid security, dispute or recordkeeping reason applies.
Is the site for children?
No. The site and forms are intended for business representatives and are not directed to anyone under 18. Do not submit information about a child.
When will the notice take effect?
The effective date will be added only when the production forms, providers and contact route are ready and the notice has completed review.
Material changes to collection, purpose, sharing, retention or rights handling will be dated before the changed practice begins where required.
